GE HealthCare (GEHC): medical imaging at 15x earnings
GE HealthCare clears the channel's three hard gates with three straight fiscal years above 15% ROE, positive free cash flow, and a roughly 15x trailing P/E, while tariffs and slower orders remain the key watch items.
The Pick
GE HealthCare Technologies Inc. (Nasdaq: GEHC) is this week's candidate. It makes imaging equipment, patient-monitoring and care devices, pharmaceutical diagnostic agents, and the software and services that connect them. The company is organized into Imaging, Advanced Visualization Solutions (AVS), Patient Care Solutions (PCS), and Pharmaceutical Diagnostics (PDx). 1
At the July 17 close, GEHC traded at $63.07 with a $28.69 billion market capitalization and a 15.10x trailing P/E. The 52-week range was $58.75-$89.77. 2
The screen passes, but the reason to study GEHC is not rapid growth. It is a cash-generative medical-technology franchise whose valuation has moved closer to a mature industrial multiple while the business is still growing modestly.
Screen Check
The ROE figures below use the labeled ROE row in Macrotrends' ratio table, which is an end-period-equity presentation rather than the average-equity convention used by some screeners. That distinction matters, so the calculation basis is explicit. TTM ROE is my calculation using TTM net income of $2.649 billion and March 31, 2026 shareholder equity of $10.680 billion. 3 4
| Hard gate | Evidence | Result |
|---|---|---|
| ROE above 15% in each of the last 3 fiscal years | FY2023 22.65%; FY2024 24.22%; FY2025 20.73%; TTM approximately 24.8% | Pass |
| Positive free cash flow | FY2023 $1.715B; FY2024 $1.554B; FY2025 $1.505B; TTM approximately $1.519B | Pass |
| Reasonable valuation | 15.10x trailing P/E; about 12.9x on the midpoint of 2026 adjusted EPS guidance; about 5.3% TTM FCF yield | Pass, with a growth caveat |
The TTM FCF estimate is reproducible: FY2025 FCF of $1.505 billion, plus Q1 2026 FCF of $112 million, less Q1 2025 FCF of $98 million. That is positive cash generation, although the annual series declined for two consecutive years before the latest quarter. 5 6
Valuation
The valuation is reasonable only relative to the company's growth and peer set, not because GEHC is a deep distressed bargain.
- Trailing P/E: 15.10x at the July 17 close. Macrotrends showed 13.83x on July 20 using a different quote and EPS snapshot, which illustrates why the timestamp and data provider should be stated. 2 7
- Forward adjusted P/E: approximately 12.9x, calculated as $63.07 divided by the midpoint of GEHC's revised 2026 adjusted EPS guidance of $4.80-$5.00. This is an adjusted, not GAAP, multiple. 6
- P/B: approximately 2.69x, calculated as the $28.69 billion market cap divided by March 31 shareholder equity of $10.680 billion. 2 4
- EV/FY2025 EBITDA: approximately 10.9x. This is my calculation: $28.69 billion equity value plus $10.127 billion of long-term debt less $2.285 billion of cash, divided by approximately $3.34 billion of FY2025 EBITDA implied by the reported 16.19% EBITDA margin and $20.625 billion of revenue. It is not a vendor-quoted TTM multiple. 4 8
- PEG: approximately 1.2x on a clearly stated forward basis. I divide the approximately 12.9x 2026 adjusted P/E by the 10.4% growth from the $4.90 midpoint of 2026 guidance to the $5.41 next-fiscal-year EPS estimate. This is a derived estimate, not a reported PEG ratio. 6 9
For a peer anchor, Abbott traded at 32.54x trailing earnings in the current MarketWatch snapshot, versus GEHC at 15.10x. GEHC is also below the iTick snapshots I checked for Medtronic at 22.43x, Stryker at 38.36x, and Boston Scientific at 18.71x. The peer comparison is directional because the businesses have different growth rates, margins, and accounting mixes; it is not a claim that GEHC deserves Abbott's multiple. 10
Business Quality
GEHC sells expensive equipment, but the economics are broader than a one-time scanner sale. Imaging systems and patient-care devices create a service relationship; software, upgrades, consumables, and pharmaceutical diagnostic agents add repeat revenue around the installed base. The result is a business that can grow without relying on a single product cycle.
That mix showed up in FY2025. Revenue rose 4.8% to $20.625 billion, with organic revenue growth of 3.5%. Imaging grew 3.8% organically, AVS grew 3.8%, and PDx grew 8.8%, while PCS declined 1.5% organically. 1
The three-year income trend is positive but uneven:
| Fiscal year | Revenue | Net income | Net margin |
|---|---|---|---|
| 2023 | $19.552B | $1.618B | 8.3% |
| 2024 | $19.672B | $2.050B | 10.4% |
| 2025 | $20.625B | $2.154B | 10.4% |
| TTM to March 31, 2026 | $20.979B | $2.649B | 12.6% |
The annual figures come from the reported financial-statement series; the TTM margin is calculated from the same source's TTM revenue and net income. 8
The moat is practical rather than mysterious: clinical workflow integration, regulatory know-how, an installed service network, and switching costs when a hospital standardizes equipment, software, training, and service contracts around one vendor. GEHC's own annual-report disclosure names Siemens Healthineers, Philips Healthcare, Canon, Mindray, and United Imaging among its primary global competitors, so the moat is real but contested. 11
Balance Sheet
The March 31 balance sheet was adequate rather than pristine. GEHC held $2.285 billion of cash against $10.127 billion of long-term debt. Total current assets of $10.426 billion versus current liabilities of $8.529 billion imply a 1.22x current ratio. Total debt divided by shareholder equity was approximately 1.60x using the latest-quarter figures. 4
Interest coverage was about 5.4x in Q1 2026 using quarterly operating income divided by interest expense. That is serviceable, but not a reason to ignore leverage. The key balance-sheet question is whether cash conversion remains strong enough to fund investment, dividends, and debt service while the company absorbs tariffs and continues to invest in new platforms.
What Could Change the Thesis
Tariffs and margin guidance. Q1 results prompted GEHC to reduce its 2026 adjusted EBIT margin outlook to 15.4%-15.7% from 15.8%-16.1%, adjusted EPS guidance to $4.80-$5.00 from $4.95-$5.15, and FCF guidance to approximately $1.6 billion from approximately $1.7 billion. Management said the Q1 PDx supplier issue was resolved, but tariffs still affected profitability. The screen should be revisited if the lower margin range becomes a further cut or if 2026 FCF falls materially below $1.6 billion. 6
PCS and order momentum. PCS was the weak FY2025 segment, with organic revenue down 1.5%. Q1 2026 total orders grew only 1.1% organically versus 10.3% in the year-ago quarter, even though backlog was $21.8 billion and book-to-bill was 1.07x. A second quarter of weak orders or a falling book-to-bill would challenge the recovery case. 6
Leverage and execution. Debt is meaningful relative to equity, and the 1.22x current ratio leaves less room for a prolonged working-capital squeeze than a net-cash balance sheet would. Investors should watch net debt, interest coverage, and FCF conversion each quarter. The SEC's Q1 filing also describes the usual medical-device exposures: product quality, regulatory requirements, reimbursement, supply chain, cybersecurity, and litigation. 12
Low insider ownership. Corporate insiders owned 0.36% of the stock; the cited filing roundup reported 21,847 insider shares bought for $1.36 million over the prior 90 days. That is a small positive signal, not meaningful alignment at the company level. 13
Short interest was 14.37 million shares, or 3.17% of float, in the MarketWatch snapshot. The level is not extreme, but it can add volatility around earnings. 2
Catalyst and Verdict
The next hard catalyst is Q2 2026 earnings on July 29. MarketWatch listed an Overweight analyst consensus and an average price target of $78.82, about 25% above the $63.07 close. The target is an external opinion, not an expected return, and it should be weighed against the lowered 2026 guidance. 9
GEHC is the pick because it passes the channel's three gates with room to spare on ROE and cash flow, while its 15x trailing P/E is below the selected medtech anchors. The trade-off is clear: this is a moderate-growth, leveraged quality business, not a clean secular-growth compounder. The screen remains valid while ROE stays above 15%, FCF stays positive and near the $1.6 billion 2026 guide, and PCS/orders stabilize. A buyer should do further work on tariff sensitivity, debt reduction, and the July 29 update before taking a position.
This is a quantitative screening article, not financial advice. Valuation and operating figures are snapshots and should be rechecked before trading.
