FDA approvals: Enspryng, Anktiva and Alcon's over-the-counter eye drop lead the Oct. 9–16 ledger

A date-bounded ledger of the FDA's October 9–16, 2026 decisions, led by the first at-home subcutaneous treatment for thyroid eye disease, a bladder-cancer expansion that rests on extrapolated evidence, and the first nonprescription apraclonidine eye drop.

Three of the FDA's headline decisions in the October 9–16 window changed how a therapy reaches the patient. Roche said the agency approved Enspryng (satralizumab) on October 16 for adults with thyroid eye disease, making it the first treatment for that condition a patient or a caregiver can inject at home. 1
That same day, the FDA approved Brytly (apraclonidine ophthalmic solution, 0.125%) for sale without a prescription to relieve eye redness, the first nonprescription drop built on apraclonidine. 2 One day earlier, UCB's Zilbrysq (zilucoplan) gained an autoinjector, giving adults with generalized myasthenia gravis a second way to give themselves the drug. 3
Two label changes carry the rest of the week. The FDA expanded Anktiva (nogapendekin alfa inbakicept-pmln) on October 14 to cover the high-grade papillary-only form of BCG-unresponsive bladder cancer. 4 An Amgen supplement approved October 9 rewrote Repatha's (evolocumab) cardiovascular claim. 5
Three boundaries set what this ledger can see. The FDA's two-week drug approvals report ran through October 16 and carried the Brytly decision, while the Enspryng approval reached Roche's announcement without appearing in the report. 6 The monthly 510(k) file was refreshed October 12 and carries item-level decisions through October 11, so no official device record exists for October 12–16. 7 The PMA decision feed was rendered October 14, and its newest entry is dated October 9. 8

New indications and label expansions

Enspryng becomes the first at-home subcutaneous treatment for thyroid eye disease

Roche said the FDA approved Enspryng on October 16 for adults with thyroid eye disease, also called Graves' ophthalmopathy. Patients or caregivers can give the injection after training from a health care provider. 1
Thyroid eye disease is an autoimmune inflammation of the tissue around and behind the eyes. It causes redness, swelling, eyelid retraction, bulging, double vision and pain, and it can threaten sight. Roche cites a United States prevalence of about 155 people per 100,000, and notes that roughly half of people with an overactive thyroid develop at least mild disease. 19
Enspryng is satralizumab, a humanised antibody that blocks the interleukin-6 receptor, the signalling pathway that drives the autoantibody attack in this disease. Chugai, part of the Roche group, developed it with antibody recycling technology that extends how long it circulates. 1
The evidence is split between two identically designed phase 3 trials, SatraGO-1 and SatraGO-2, which randomised 258 adults across 19 countries one to one to Enspryng or placebo over 72 weeks. The primary endpoint was the share of patients with active, moderate-to-severe disease achieving at least a 2 mm reduction in proptosis, or eye bulging, at week 24. 19
SatraGO-2 met the endpoint: 52.9% of patients on Enspryng had a proptosis response against 23.4% on placebo, and double vision improved in 60.7% against 25.8%. SatraGO-1 did not reach statistical significance, with responses of 49% against 31.2%, which the investigators attributed to an unexpectedly high placebo response. 9 Roche reported no new safety signals, and a profile consistent with the drug's record in neuromyelitis optica spectrum disorder. Dosing is at weeks 0, 2 and 4, then every four weeks. 1
The approval came with priority review and marks the second indication for Enspryng, after neuromyelitis optica spectrum disorder in 2020. 1

Commercial read

The commercial case rests on the route, not on a demonstrated efficacy edge. Two disease-specific therapies were already approved for thyroid eye disease, Tepezza (teprotumumab) in 2020 and more recently Lumvoa (veligrotug), both given by intravenous infusion. No head-to-head comparison with either has been reported, and the trials behind them differed in design and population. 9
Differences in how the drugs are given are where the choice now sits. Tepezza's pivotal trial dosed eight infusions over 24 weeks and produced a 71% response on the same 2 mm threshold against 20% on placebo, but the comparison to Enspryng crosses trials rather than patients. 9
Roche disclosed neither pricing nor launch timing at approval, and the drug is already used in roughly 90 countries for neuromyelitis optica spectrum disorder. The next read on the franchise comes in January 2027, when the FDA is due to decide a supplemental application in myelin oligodendrocyte glycoprotein antibody-associated disease, a condition with no approved therapy. 19

Anktiva's bladder cancer label widens by extrapolation

The FDA expanded Anktiva on October 14 to adults with high-grade papillary-only bladder cancer that has not grown into the muscle and has not responded to BCG. Anktiva is the first FDA-approved treatment specifically for patients with BCG-unresponsive, high-grade papillary-only disease. It is given with BCG directly into the bladder through a catheter. 4
Bladder cancer that has not reached the muscle layer comes in two shapes. Carcinoma in situ forms flat patches along the bladder lining, while papillary tumors grow from the lining as small finger-like projections. High-grade cancers in either shape are more likely to grow and spread. 4
The new coverage rests on no new trial. The FDA based it on evidence supporting the earlier approval in carcinoma in situ, a single-arm study of 77 adults in Cohort A of QUILT-3.032 (NCT03022825), where 62% achieved a complete response. Of those responders, 58% held the response for at least 12 months and 40% for at least 24 months. The agency judged that the two histologic types are aggressive urothelial carcinomas that frequently coexist and can be difficult to tell apart under a microscope, and it applied the carcinoma in situ result to the papillary-only group. 410
Dosing is 400 micrograms intravesically with BCG once a week for six weeks, with a second induction course allowed if residual disease remains at month three. Maintenance runs to a maximum of 37 months and stops for disease persistence after the second induction, recurrence or progression, or unacceptable toxicity. 10
Adverse reactions in at least 15% of patients, including laboratory abnormalities, were increased creatinine, painful urination, blood in urine, urinary frequency, urgency, urinary tract infection, increased potassium, musculoskeletal pain, chills and fever. The prescribing information warns that delaying surgery to remove all or part of the bladder can allow the cancer to spread, which can be fatal. 4

Commercial read

The read here is about the route the agency used as much as the drug. Agency reviewers accepted evidence generated in one form of the disease to support a claim in another, and the FDA said it is working toward finalising its guidance on developing drugs for BCG-unresponsive non-muscle invasive bladder cancer, which is intended to discuss that approach. 410
For developers running trials in this disease, a finalised guidance would set the terms for borrowing carcinoma in situ evidence, which shortens the path to a papillary-only claim. R. Angelo de Claro, who directs the FDA's Oncology Center of Excellence, framed the Anktiva review that way, saying the similarities between the cancers let the agency build on the carcinoma in situ results. 4
Anktiva keeps the BCG-unresponsive setting to itself among approved bladder-cancer immunotherapies. The nearest recent entrant, durvalumab (Imfinzi) with BCG, was approved on May 28, 2026 for BCG-naïve high-risk disease, a different point in the treatment sequence. 15

Repatha's cardiovascular claim widens past uncontrolled LDL cholesterol

An Amgen supplement to Repatha approved October 9 changed the drug's first indication. The prescribing information revised that month states the claim as reducing the risk of major adverse cardiovascular events, listed as cardiovascular death, myocardial infarction, stroke, unstable angina requiring hospitalisation, coronary revascularisation or any ischaemia-driven arterial revascularisation, in adults at increased risk for those events. 11
The earlier wording, added when the FDA broadened the drug's use in August 2025, tied the same risk reduction to adults whose LDL cholesterol was uncontrolled. 16 The October label carries no LDL cholesterol requirement in that claim. 11
FDA's own approval record classifies the October 9 action as an efficacy supplement for a new indication. 5 The LDL cholesterol and familial hypercholesterolaemia uses of the drug carry over unchanged, for adults with hypercholesterolaemia and for adults and children aged 10 and older with heterozygous or homozygous familial hypercholesterolaemia. 11

Commercial read

Amgen says roughly 9 million patients have used the drug worldwide, and that it is approved in 75 countries. 17
The supporting trial is VESALIUS-CV, which enrolled adults at high cardiovascular risk with no prior heart attack or stroke and tested evolocumab on top of statins. Amgen presented a pre-specified analysis at the European Society of Cardiology Congress in August 2026 showing a reduction in all-cause death, and published the results in Circulation. The European Commission expanded the drug's European indication on the strength of that trial in August 2026. 17
Payer policy is where the broadened wording lands. A claim that covers adults at increased risk, without requiring that their LDL cholesterol is uncontrolled, gives Amgen a wider label to argue from while leaving prior authorisation criteria to the plans.

New formulations and delivery options

Brytly puts apraclonidine on the shelf without a prescription

The FDA approved Brytly on October 16 as a nonprescription treatment for eye redness caused by minor irritations. It is the first nonprescription eye drop containing apraclonidine, and it is approved for adults and children 12 and older at no more than two doses a day. 2
Eye redness usually comes from swollen or dilated blood vessels on the surface of the eye, triggered by things like dryness or dust. Apraclonidine works on those vessels; higher concentrations of the same drug stay behind prescription for reducing eye pressure in glaucoma and around certain laser eye surgery. 2
The agency directed consumers to remove contact lenses before use and wait at least ten minutes before reinserting them, and to stop using the drop and see a doctor for eye pain, vision changes, redness that continues past three days or symptoms that worsen. Alcon holds the approval. 2 FDA's application record lists the decision as an original application approved under priority review. 12

Commercial read

An over-the-counter approval is a different business from a prescription launch, because the revenue sits in retail volume rather than in reimbursement. The FDA placed the decision inside its stated push to expand access to nonprescription medicines, and Mike Davis, who directs the agency's Center for Drug Evaluation and Research, described broadening nonprescription access as a priority. 2
For Alcon the approval adds a differentiated molecule to a category that has been served by older vasoconstrictor drops, and it gives the company a consumer product it can market without a clinician in the loop. The label's twice-daily ceiling and its stop-use warnings are the practical limits consumers will meet on the shelf.

Zilbrysq's autoinjector gives myasthenia gravis patients a second self-injection option

FDA's application record shows a supplement to Zilbrysq (zilucoplan) approved on October 15, and UCB said the agency cleared an autoinjector version, making the macrocyclic peptide C5 inhibitor the first treatment of its kind available as a once-daily, self-administered subcutaneous autoinjector for adults with generalized myasthenia gravis. 313
Zilucoplan has been available since 2023 as a prefilled syringe for adults with the disease who are acetylcholine receptor antibody positive. The autoinjector delivers a pre-set dose when pressed against the skin, and UCB expects it in the market in the first quarter of 2027. 13
The agency accepted the device on bioequivalence and usability rather than on a new efficacy trial. In a phase 1 crossover study in 14 healthy volunteers, the autoinjector matched the prefilled syringe; in a phase 3b study in 31 patients, complete dose delivery was achieved across 99.8% of 449 autoinjector uses, with injection-site pain in three patients. A human factors study of 100 participants found 90% completed an injection on the first attempt without training. 13
Zilbrysq carries a boxed warning for serious meningococcal infections and is dispensed only through a restricted programme under a risk evaluation and mitigation strategy, with meningococcal vaccination recommended at least two weeks before the first dose. 13

Commercial read

Generalized myasthenia gravis is rare, with a global prevalence of 150 to 350 cases per million people, so the commercial contest is fought over share among a small set of patients rather than over market creation. 13
The device matters because it removes a step rather than a dose. Patients taking the prefilled syringe face the same daily drug in a form that takes more handling, and UCB already sells a second myasthenia gravis therapy, Rystiggo (rozanolixizumab-noli), which is given by subcutaneous infusion. Two self-administered products with different mechanisms of action give the company two positions to defend in the same clinic. 13

For three of the supplements above, FDA's public records state the action and its classification without describing what changed. The Cosentyx and Enflonsia labels were not posted when their records were read, and the Olumiant record carries a classification of a labeling change with clinical data alongside a note that its label is unavailable. Those changes stay open until the labels appear. 192122

No new original PMA approvals, and twenty supplements

The PMA decision feed carried no original approvals and twenty supplements decided on October 8 and 9, its newest entries when the feed was rendered on October 14. 8 The feed lists each of those twenty as a PMA number and a decision date, so what each supplement changed rests on its own device record.

What matters next

  • The device ledger for October 12 through 16. The monthly file stops at October 11 and the PMA feed at October 9, so the next issue should pick up whatever those surfaces add for the closing days of this window.
  • Enspryng against the infusion therapies. Whether payers and clinicians move thyroid eye disease patients to a home injection will show in formulary placement, and Roche faces its next decision in January 2027 in MOGAD.
  • Anktiva's papillary-only launch and the FDA guidance. The extrapolation approach becomes a precedent for other sponsors once the agency finalises its BCG-unresponsive guidance.
  • Repatha's broader claim in practice. The label now covers adults at increased cardiovascular risk without an LDL cholesterol condition, and prior authorisation criteria will decide how much of that reach is real.
  • The Cosentyx, Enflonsia and Olumiant labels. Three supplements of this window were approved with their labels unposted, so their specific changes stay open until FDA publishes them.
