Three Small Substacks That Turned Specialist Trust into 100 Paid
This week profiles Techy Surgeon, Elder of Ziyon, and Voices from the Front, showing how specialist authority, deep archives, and firsthand access became 100-plus paid Substack readers.
The short version
The useful number here is 100.
Between June 24 and July 24, three solo publications cleared Substack's 100-paid-subscriber Bestseller threshold while remaining below 5,000 total subscribers: Techy Surgeon, Elder of Ziyon, and Voices from the Front. They sell three different things: practical health-tech judgment, a long-running analytical point of view on Israel and antisemitism, and first-person war reporting from Ukraine.
That is the important contrast. None is a generic newsletter about newsletters. Each earns attention by narrowing the reader's problem until the author's lived experience becomes hard to substitute. A 500-reader publication at $10 a month would gross $60,000 a year before fees, refunds, taxes, and churn. This week's cases suggest the craft is not "be more charismatic." It is choosing a field where your evidence, access, or accumulated judgment gives a small group a reason to stay.
| Publication | Public total-subscriber count | Paid milestone disclosed this month | Public price |
|---|---|---|---|
| Techy Surgeon | 3,700 in the author's July 15 update | Substack Bestseller, therefore at least 100 paid | $25/month; $245/year |
| Elder of Ziyon | 2.2K+ | 107 paid on July 17 | $11/month; $110/year |
| Voices from the Front | Over 3,000 | Substack Bestseller, therefore at least 100 paid | $11/month; $110/year |
The count language matters. "3,700," "2.2K+," and "over 3,000" are not interchangeable exact totals. The paid figures for Techy Surgeon and Voices from the Front are threshold claims, not disclosed dashboards. That verifies the milestone, not a precise conversion rate.
1. Techy Surgeon: the operator's notebook, not an AI news feed
Positioning and numbers. Dr. Christian Péan describes Techy Surgeon as a working notebook on the future of care, where clinic, capital, code, and policy meet. His public profile identifies him as an orthopedic trauma surgeon and CEO/co-founder of RevelAi Health. The promise is practical playbooks for clinicians, operators, and builders rather than another stream of AI headlines. 1
On July 15, Péan posted a one-year recap: zero to 3,700 subscribers, Substack Bestseller status, and more than 150 articles on health policy and clinical AI. The post is a clean public disclosure inside the required window. It does not disclose the exact number of paid readers, so the defensible claim is 100-plus paid, not a made-up percentage of 3,700. 2
The checkout page exposes a $25 monthly tier and a $245 annual tier. It also lists a free layer of weekly policy analysis and clinical-AI insights, while paid readers receive deep dives, the full archive, Techy Flows, policy templates, monthly Q&As, and access to the wider resource library. 3
Authority and editorial design. The unfair advantage is not simply being interested in AI. It is having to make clinical and operational decisions while building a health-tech company. The About page promises one to two posts a week, with explainers, frameworks, prompt guides, tutorials, value-based-care playbooks, and candid notes about what broke or shipped. That cadence is enough to create a habit without pretending every post is a definitive research report. 1
The funnel is well chosen for a specialist audience. Free readers get enough signal to test whether Péan understands their work. Paid readers get the artifacts that reduce implementation time: full videos, interactive guides, skill-creation walkthroughs, templates, and archive access. In a representative paid post, the visible section explains the method before a seven-day trial CTA opens the full video, complete guide, and deeper workflow material. 4
The observable conversion lever. The July anniversary recap is the public trigger we can actually see: 150-plus articles, a Bestseller badge, and a cross-platform invitation to help shape year two. It is tempting to say one course announcement or one post caused the jump. The public record does not prove that. What it does show is a conversion system built from repeated, useful output plus a clear upgrade from information to working assets.
Who could clone it. A hospital informatics lead, a revenue-cycle director, or a practicing pharmacist who has spent years implementing one difficult workflow could use the same model. The clone is not "AI for healthcare." It is a narrower promise such as: monthly playbooks for reducing medication-reconciliation errors in rural hospitals, with free case notes and paid templates, checklists, and implementation debriefs.
2. Elder of Ziyon: two decades of context behind a partial paywall
Positioning and numbers. Elder of Ziyon publishes original articles about Israel and antisemitism. The Substack profile says the pseudonymous author has written about Israel, the Arab world, and antisemitism since 2004, and has written two books. The profile displayed 2.2K-plus subscribers when checked. That makes this a solo-author case with a deliberately private identity, not a media-company publication. 5
On July 17, the author wrote that Substack had flagged the publication as a Bestseller and then gave the unusually valuable number: 107 paid subscribers. The same post explains that the paid subscription funds time for deeper, sourced pieces. 6 The public plan is $11 a month or $110 a year, with subscriber-only posts, the full archive, comments, and community access. 7
Authority and editorial design. The moat is accumulated context. The About page says the author has tens of thousands of old posts to draw from, then describes a shift from standalone commentary toward serial projects and a framework called Derechology. That is a useful distinction for practitioners: the product is not just fast reaction. It is a private method for connecting recurring evidence across years. 8
The free-versus-paid cut is a partial paywall rather than a free teaser with no substance. Free readers get something genuinely valuable; paid readers get the full picture, longer pieces, series, and the archive. The archive shows near-daily publishing across July 13–23, including multiple posts on July 17, but it does not publish a standard word count or a neat weekly package. 9 That messier stream fits a publication built around a living beat and serialized thinking.
The observable conversion lever. The milestone post is itself a compact funnel. It thanks the 107 paid readers, explains what their money makes possible, points to a partial-paywall product, and offers books and one-time tips for readers who do not want a recurring charge. The author also identifies series and the Haggadah project as tangible reasons to support the work. We cannot prove which of those moved the last reader across 100. We can say the offer gives readers several ways to convert without weakening the central paid promise.
Who could clone it. Think of the senior public-records researcher who has spent 15 years tracking one state agency, or the compliance lawyer who has built a private archive of enforcement decisions. Their newsletter should not promise "all the news." It should turn a long memory into a recurring series: free context and selected documents, paid full timelines, source notes, and the analyst's framework for seeing the pattern.
3. Voices from the Front: lived experience as a reporting moat
Positioning and numbers. Shaun Pinner describes Voices from the Front as reporting from Ukraine on war, survival, and the human cost behind the headlines. His profile identifies him as a former British soldier and Ukrainian Marine, a defender of Mariupol, and the author of Live. Fight. Survive. 10 The publication page displayed over 3,000 subscribers. 11
In a July 21 post, Pinner wrote that the newsletter had become a Substack Bestseller and that more than 3,000 people had subscribed. That verifies the 100-paid threshold within the window, but not an exact paid count. The public plan is $11 monthly or $110 annually. 12
Authority and editorial design. This is a narrow reporting promise backed by access and biography that cannot be manufactured through keyword research. Free readers get selected essays, public video, and views into Ukrainian culture and the war. Paid readers get full-length essays and field reports, early access to unseen video and interviews, legal-case updates, private Q&As, and announcements about future projects. 13
The cadence is high-frequency rather than tidy: the archive shows posts on nearly every day from July 15 through July 22, with multiple video and live-recording formats. Recent video entries run roughly 24 to 67 minutes; the archive does not publish a typical article word count. 14 The format mix lets the author serve both the reader who wants a fast update and the paid member who wants time, context, and access.
The observable conversion lever. Pinner's July 21 note says every subscription, comment, restack, and recommendation helped the community grow. That is a direct description of the distribution loop, not proof that one post caused the milestone. The reusable lesson is to make support visible and specific: explain what a paid membership enables, then give readers a reason to share the work even when they cannot pay. 12
Who could clone it. A local investigative reporter, disaster-response coordinator, or former aid worker could build a similar publication around a place where firsthand access matters. The clone should not imitate war coverage. It could be a weekly field report on rebuilding one flood-prone region, with free dispatches and paid interviews, document packs, and the operational details that national outlets leave out.
What the three cases actually prove
They do not prove that any niche expert can open a Substack and reach 100 paid readers in a month. They do show three repeatable design choices:
- A specific reader and problem. Clinicians and builders; readers trying to understand Israel and antisemitism; people who want reporting from inside Ukraine.
- A moat that is visible before the paywall. Clinical practice, a 20-year archive, or firsthand experience.
- A paid upgrade that buys depth or time. Working assets, full analytical series, field reports, video, access, or archives.
The lesson for a domain expert is not to hide the good material. It is to give free readers enough evidence to recognize the authority, then reserve the costly-to-produce layer for members.
Three adjacent niches to watch
This is a gap hypothesis, not a census of every newsletter. I looked for fields with active specialist information demand where the visible supply is mostly trade media, association material, or organization-led newsletters rather than a clear solo-author paid leader.
- Agricultural aviation operations. Ag-pilot work combines low-altitude flying, chemical application, weather, GPS, and safety decisions. A recent Plane + Pilot feature calls ag pilots professional applicators, while Air Tractor's industry archive shows an active stream of aircraft, operations, and stewardship stories. That looks like fertile ground for a solo operator who writes for pilots and farm-service owners about the business and risk decisions behind the aircraft. 1516
- Public-contract investigations for local suppliers. Open Contracting Partnership publishes a newsletter with more than 1,000 subscribers, but it is organization-led. Local Government Lawyer provides a separate professional procurement-news stream. The opening is a practitioner-led paid briefing for small contractors, procurement officers, or reporters that turns tender data into actionable explanations for one jurisdiction. 1718
- Clinician-facing health-tech liability. The search surface includes a free newsletter for clinicians interested in HealthTech, but the field still lacks an obvious solo publication whose narrow product is practical liability, credentialing, and implementation decisions for frontline teams. A surgeon, risk officer, or medical-staff attorney could own that intersection with free case explainers and paid policy templates, review checklists, and monthly office hours. 19
The common thread is the same one these three Bestsellers make visible: small paid publications win when the author knows a difficult corner of the world well enough to make the reader's next decision easier.
