Your migraine plan starts before the pain peaks. Here's what actually helps.
A plain-language migraine guide covering first-hour relief, medication-overuse traps, and the headache symptoms that need urgent care.
The 60-second version
A migraine is more than a bad headache: it often comes with throbbing pain, nausea, and strong sensitivity to light or sound. Most attacks last 4 hours to 3 days. The best first move is usually boring but effective: get somewhere dark and quiet, drink some water, use a cold pack, and take a safe, label-directed pain reliever early if you normally use one. 1 2
A sudden explosive headache, your worst-ever headache, or a headache with new weakness, confusion, speech trouble, vision loss, fever, or a stiff neck is not a "wait and see" situation. Get emergency help. 3
What a migraine usually feels like
Migraine pain is often pulsing or throbbing and may be worse when you move around. It can be on one side or both sides of your head. Nausea, vomiting, and a strong reaction to light or sound are common. The pattern matters more than whether the pain is on a particular side. 1
Some people get an aura before or during the headache. That can look like flashing lights, zigzag lines, blind spots, tingling, dizziness, or trouble speaking. A typical aura should resolve within an hour. If this is the first time you have had those symptoms, they are much more severe than usual, or they do not clear, treat it as a reason for urgent medical advice rather than assuming it is "just your migraine." 1 3
You can have a migraine without every classic symptom. You can also have a serious problem that feels like a migraine. A familiar pattern that builds gradually and improves with your usual treatment is less alarming than a brand-new, sudden, or rapidly changing pattern, but a checklist cannot diagnose you.
What to do when the pain starts
- Cut the stimulation. Turn down the lights, leave the noisy room, put your phone away, and lie down if you can. A cold towel or cold pack on your forehead or neck may help. Rest or sleep is reasonable if you are otherwise safe. 2
- Sip fluids. Migraine plus vomiting is a fast route to dehydration. Take small, regular sips instead of forcing a huge drink. If you have not eaten, try a small, plain snack if your stomach allows it.
- Use your usual over-the-counter option early, if it is safe for you. MedlinePlus lists acetaminophen, ibuprofen, naproxen, and aspirin as possible options for some people. Follow the package directions, check the active ingredients in combination cold or flu products, and do not take a medicine you have been told to avoid. Ibuprofen, naproxen, and aspirin can irritate the stomach; acetaminophen can be dangerous in excess or when mixed with heavy alcohol use. 2
- Do not try to power through with alcohol or an energy drink. Both can make an attack harder to manage for some people. If caffeine is one of your regular triggers, do not use a sudden extra dose as a rescue plan.
If you already have a prescription migraine medicine, use it exactly as directed. If your normal treatment suddenly stops working, that change is a reason to contact a clinician instead of repeatedly adding more tablets. 2
The painkiller trap
Taking an acute headache medicine too often can start increasing the number of headache days. This is called medication-overuse headache, and it can happen with over-the-counter painkillers as well as prescription rescue medicines. The American Headache Society says people having headaches on 4 or more days per month should be assessed for preventive options; MedlinePlus advises contacting a healthcare provider if you need pain medicine more than 3 days a week. 4 2
A simple note in your phone can make this easier to spot. Record the start time, how long the attack lasted, your main symptoms, what you took, how much you slept, and whether you skipped food or fluids. You are looking for a pattern, not trying to build a perfect spreadsheet.
When to handle it at home, book care, or get help now
Home care is reasonable when
The headache gradually builds, feels like your usual migraine pattern, and starts improving with rest and your usual safe treatment. Keep drinking fluids, avoid driving if your vision or alertness is affected, and give yourself time to recover. A migraine attack can last up to 72 hours. 1
Arrange a clinician visit soon when
- This is your first migraine-like headache, or your pattern is changing.
- Your usual treatment no longer works or causes troublesome side effects.
- You are getting headaches frequently, especially 4 or more days a month.
- You need acute pain medicine more than 3 days a week.
- The attack lasts longer than 72 hours.
- You are pregnant or recently gave birth and have a new or severe headache. 2 1
Get emergency help now when
- The pain arrives suddenly and explosively, or it is the worst headache you have ever had.
- You have new weakness, numbness, trouble speaking, confusion, fainting, loss of balance, or a major change in vision.
- The headache comes with fever and a stiff neck, follows a head injury, or causes a seizure.
- You have severe pain in one red eye.
- An aura lasts longer than an hour or feels unlike your previous auras. 3 1
The useful rule is simple: treat a familiar, gradual attack early, but do not use a familiar migraine label to explain away a sudden or neurologically unusual headache.
The takeaway
Put a cold pack, water bottle, and the medication you safely use in the same easy-to-reach place. Then track how often you need that rescue plan. If the headaches are new, changing, unusually long, or happening often, the next useful step is not another random painkiller. It is a proper evaluation and a plan to prevent the next one.
This article is general health information, not a diagnosis. If a headache feels dangerous or different from your normal pattern, seek medical care.
