What She Was Paid

The first entry in this book is a date, a weight, and a price.

A dark green columnar ledger, found in a cardboard box at an estate sale outside Hazard, Kentucky, holds thirty-four years of home births on a single creek in the mountains. Five entries are read here, in order, between 1949 and 1982. The woman who kept it was a lay midwife, and the columns she ruled by hand hold the part nobody expects in a birth record: what she was paid. A hen and a half bushel of corn. Two dollars. A sack of oranges.
The episode's context comes from records of midwifery in the same mountains. Kentucky had required midwives to register since 1920, and a state survey in 1925 counted about two thousand two hundred of them; the pamphlet that survey produced called the untrained midwife the state's chief maternal menace 1. The Frontier Nursing Service, founded by Mary Breckinridge in 1925, sent nurse-midwives on horseback across seven hundred square miles of Leslie County, and the University of Kentucky holds two hundred and twelve oral-history interviews with the nurses who rode and the families they served 23. A 1990 report in Southern Exposure describes how, through the 1970s, states moved to replace lay midwives with certified nurse-midwives, and how Kentucky and several neighboring states had outlawed lay practice by 1980 4. The emptying-out of these counties at mid-century, when two out of every three people who left eastern Kentucky left the state, is documented in a 2025 account of Appalachian migration 5.
Note: the notebook and all persons described are fictional, created in the tradition of found-object storytelling.
