The lawsuit: The case, Clausner et al. v. American Multispecialty Group, Inc. d/b/a Esse Health, alleges that an April 2025 cyberattack exposed potentially sensitive personal and health information. Esse Health denies wrongdoing; the case is pending in the 22nd Judicial Circuit Court of St. Louis City, Missouri, Case No. 2622-CC00414. 1
Who may qualify: People identified in Esse Health’s records as potentially affected by the April 2025 incident. The administrator says class members may already have received a direct notice. The public site does not turn this into an open-ended claim for every Esse Health patient.
What it is worth: About $50 in cash, with the final amount able to move up or down depending on valid claims. An enrollment code for medical identity protection is also described as an automatic benefit after final approval. The cash option does not require receipts or other loss documents. 3
How to file: Use the official Esse Health claim form. Have the notice or enrollment information ready, complete the cash-payment choice, and submit. That is roughly three actions; the administrator does not publish a formal step count or say that a separate account is required. Who may qualify: US residents whose information was potentially affected and who received the incident notice. This is not a general claim for anyone who has used a staffing service. The settlement offers a $50 alternative cash payment without proof, while documented ordinary and extraordinary losses require supporting records.
How to file: Use the official Triage claim form. The practical path is: enter the notice details, choose the no-proof cash option, and submit, or download and mail the form. Allow about five minutes if your notice is in hand. Claims are due August 10, 2026; the final approval hearing is listed for August 18. 4

Legends Hospitality: estimated $200 by August 19

The Cook et al. v. Advanced Recovery Equipment & Supplies: up to $50 by August 24

Edri et al. v. Advanced Recovery Equipment & Supplies, LLC, Case No. 529489/2024, is pending in New York Supreme Court, Kings County, over an October 2024 data-security incident. US residents whose personal information was compromised may qualify. The proposed settlement offers up to $50 without proof, plus two years of one-bureau credit monitoring; higher amounts are reserved for documented losses and are outside this tracker’s low-friction range. 7
File through the official claim portal. You will need the login and password in the notice. The practical sequence is login, select the $50 alternative, and submit. Claims are due August 24, 2026. The final approval hearing is listed for September 24, and the payment is not guaranteed until approval and appeals are complete. 8

McKenzie Health System: $50 by August 24

The In re: McKenzie Memorial Hospital d/b/a McKenzie Health System 2025 Data Breach Litigation, Case No. 2025-40999-CZ, concerns an April 2025 attack that may have exposed medical and identifying information. The case is pending in the Circuit Court for Sanilac County, Michigan. Eligible people are living US individuals whose private information was potentially compromised and who received notice. 9
The no-proof alternative is $50; documented-loss claims can reach higher amounts but require records. The official claim form is the shortest route. Plan on notice login, cash-option selection, and submission, or mail the PDF. The deadline is August 24, 2026 and the final approval hearing is October 6. 10

Call-On-Doc: up to $20 by August 29

Lucas v. Call-On-Doc, Inc., Case No. 2026LA000403, is pending in the Circuit Court of Will County, Illinois. The lawsuit alleges that Call-On-Doc used third-party tracking pixels on its pages in violation of California privacy laws; the company denies wrongdoing. The class covers people who created a Call-On-Doc account on or after November 7, 2023, or registered users who began booking medical care on or after November 7, 2021, with the settlement notice directing the eligible group. 11
The cash payment is up to $20, with no proof requirement stated for the ordinary claim. Use the official claim form, enter the notice information, complete the form, and submit. Claims are due August 29, 2026. The final approval hearing is August 31. 12

YouTube TV: about $92 by August 30

The Dutcher et al. v. Google LLC d/b/a YouTube, Case No. 20CV366905, alleges that YouTube TV did not satisfy California automatic-renewal disclosure rules. The case is pending in Santa Clara County Superior Court, and the defendants deny the allegations. The settlement is for California residents who paid for at least one renewal term of a YouTube TV subscription purchased through YouTube billing between February 1, 2017 and October 29, 2021, for personal, family, or household use. 13
The $7.5 million fund is divided pro rata. The administrator’s current estimate is about $92.26, not a promised flat payment, and the claim requires no purchase receipt. You will need the settlement email or notice to use the official claim form. Allow about five minutes to authenticate, complete, and submit. The deadline is August 30, 2026; final approval is scheduled for October 15. 14

  3. One notice, one form: For Legends, Advanced Recovery, McKenzie, Triage, or Call-On-Doc, file only the no-proof alternative if you qualify. Do not claim the larger documented-loss tier without actual records.
The common denominator is uncertainty: these are proposed settlements, and pro-rata or alternative payments can move when the administrator counts valid claims. A five-minute filing is reasonable when your notice and eligibility are clear. It is not worth turning an unverified email, old account, or vague memory into a claim.
