Ebola Bundibugyo: case count surges, Uganda seals border, US screening reaches four airports

The week's most significant development was a step-change in the Bundibugyo Ebola case count. WHO Disease Outbreak News 605 (DON605), published May 29, reported 125 confirmed cases and 906 suspected cases in DRC as of May 27, along with 17 confirmed deaths and 223 suspected deaths. Compared to DON603 (May 21), this represents 49 new confirmed cases and 8 new confirmed deaths in 8 days. 2 Uganda reported 9 confirmed cases (8 in Kampala, 1 in Wakiso) and 1 confirmed death. 2
A note on competing numbers: Africa CDC's May 28 briefing reported higher totals — 1,077 suspected cases, 246 possible deaths, and 129 DRC confirmed + 8 Uganda confirmed. The ECDC clarified that DRC authorities performed a data revision around May 27–28, removing duplicate or misclassified records and reclassifying some suspected cases as confirmed, which is the most likely explanation for the divergence. 3 Readers should use WHO DON605 figures as the more recently revised reference. The CDC situation summary as of May 29 is consistent with WHO on DRC numbers; it records 2 confirmed deaths in Uganda vs. WHO's 1 — a minor discrepancy under investigation. 1
The outbreak remains concentrated in DRC's Ituri Province, which accounts for 88% of confirmed DRC cases. The four most affected health zones are Bunia (37 confirmed), Rwampara (33), Mongbwalu (20), and Nyankunde (10). 2 DRC's test positivity rate stands at 19.2% based on 648 samples processed; more than 100 samples remain pending, meaning the true rate could be higher. 2 A total of 16 healthcare workers have been confirmed infected in DRC. 2
Uganda closes its border. On May 27, Uganda's Ebola Task Force — led by Vice President Jesca Alupo — ordered the immediate closure of Uganda's border with DRC. 4 The trigger was the discovery that Congolese patients had crossed the border seeking care before the outbreak was officially declared on May 15, exposing Ugandan healthcare workers. Dr. Diana Atwine, Permanent Secretary of Uganda's Ministry of Health, said: "They have families, and so the number has been increasing." 4 WHO had consistently recommended against border closures in prior DONs, noting that restrictions tend to push travelers through informal crossing points where monitoring is weaker.
US entry screening expands to four airports. The 30-day PHSA travel restriction order issued May 18 under 42 U.S.C. §§ 265 and 268 remains in force and is on track to expire around June 17. 5 This week brought two significant expansions:
  • May 22: A HHS interim final rule amended 42 CFR Part 71.40 to extend the entry suspension to US lawful permanent residents (green card holders), who had been exempt from the original order. 6
  • May 22–28: CBP sequentially added Atlanta (ATL, May 22), Houston Intercontinental (IAH, effective May 26), and New York JFK (effective May 28) to the designated entry airports alongside Dulles (IAD). Travelers from DRC, Uganda, or South Sudan who transited the past 21 days must now enter through one of these four airports. 7
CDC continues to assess the immediate risk to the US public as low. 1
American surgeon remains stable in Berlin. The US surgeon infected in DRC on May 11 — who developed symptoms May 16 and was laboratory-confirmed May 20 — is receiving treatment in a high-level isolation unit in Berlin and is in stable condition as of May 29. High-risk contacts have been transferred to Germany and the Czech Republic and remain asymptomatic. No Ebola cases have been confirmed inside the United States. 1
Vaccine pledge and Russia's claim. Africa CDC Director-General Jean Kaseya pledged at a May 28 online briefing that Africa CDC will develop a Bundibugyo vaccine and treatment by end of 2026: "What we can tell you for sure, by the end of this year, 2026, Africa CDC will make sure that we have a vaccine and medicine against Bundibugyo." 8 Kaseya also noted that Russia's health ministry contacted Africa CDC the day before claiming it has a candidate vaccine, though an Africa CDC team member clarified that the Russian candidate targets the Zaire strain; whether it would offer cross-protection against Bundibugyo is to be discussed with Moscow's Gamaleya National Research Center. 8 WHO DON605 states explicitly: there are no approved vaccines or specific treatments for Bundibugyo virus disease. 2
Eleven African nations at elevated risk. Africa CDC identified 11 countries at high risk of spillover: South Sudan, Rwanda, Kenya, Zambia, Central African Republic, Tanzania, Ethiopia, Angola, Republic of Congo, Burundi, and Somalia. 9 Kaseya described the situation as still in the "active phase" with case numbers expected to rise. 9
Texas DSHS issued a separate alert on May 20 reminding Texas clinicians to maintain vigilance for Ebola symptoms, noting that DRC's national soccer team is scheduled to train in Houston and play a World Cup group-stage match there next month. DSHS is coordinating with CDC, FIFA, and local health departments. 10
What you should do:
  • Travelers: Anyone who has been in DRC, Uganda, or South Sudan in the past 21 days must enter the US through IAD, ATL, IAH, or JFK. If you returned before May 28 through a different airport, contact your local health department for guidance on completing the 21-day monitoring protocol.
  • LPRs with recent travel: If you are a green card holder who traveled to any of the three countries within the past 21 days, the May 22 rule change means you are now subject to the entry restriction. Contact DHS/CBP or a legal advisor if this affects pending travel.
  • Self-monitoring: Watch for fever, severe headache, muscle pain, unexplained bleeding, vomiting, or diarrhea. If symptoms appear within 21 days of travel to affected areas, isolate and call a healthcare provider before going in person.
  • Clinicians: GeneXpert does not detect Bundibugyo virus. Use BDBV-capable PCR platforms and contact CDC Emergency Operations Center (770-488-7100) for testing logistics. Bundibugyo transmits only through direct contact with infected body fluids — no airborne spread.

Hantavirus (Andes virus): 13 cases, Rt below 1, no US cases confirmed

WHO DON604, published May 28, updated the MV Hondius cruise ship Andes virus outbreak to 13 cases (11 laboratory-confirmed + 2 probable) and 3 deaths (case fatality rate 23%). 11 Since the previous DON (May 13), Canada, the Netherlands, and Spain each added one new confirmed case among passengers or crew. The 1 previously inconclusive US case was re-tested and excluded on May 15 — no US confirmed cases. 11
Whole-genome sequencing of cases shows ≤1 single-nucleotide polymorphism difference across samples, consistent with a single zoonotic spillover event aboard the ship rather than ongoing human-to-human chains. 11 The effective reproduction number Rt is estimated at 0.7 as of May 22, below the threshold for self-sustaining spread. 11 Over 600 contacts in 32 countries remain under follow-up; 53% are classified high-risk (42-day quarantine period) and 47% low-risk (self-monitoring). 11
CDC's HAN #529 (issued May 18) — the current standing guidance — describes Andes virus as requiring prolonged close contact for human-to-human transmission; the cruise ship environment's shared enclosed spaces elevated risk. CDC HAN #529 states that "as of May 18, no confirmed cases of Andes virus associated with the outbreak on the cruise ship have been reported in the United States. Therefore, the overall risk to the American public is still considered extremely low at this time." 12
What you should do:
  • If you traveled on MV Hondius or provided care to a Hondius passenger, contact your local health department. High-risk contacts remain under 42-day monitoring.
  • Clinicians: Andes virus-specific assays will not detect New World hantaviruses endemic to the US (such as Sin Nombre virus). Order appropriate panels based on exposure history and contact CDC's Viral Special Pathogens Branch (EOC: 770-488-7100) before testing.

Mpox: US Clade I count reaches 23, Clade Ib persists in Western Europe

CDC's May 26 situation update recorded 23 laboratory-confirmed Clade I mpox cases in the US since November 2024 — an increase of 1 from the prior week's count of 22. All 23 remain travel-related: linked to Central or Eastern Africa, Western European countries with active Clade Ib transmission, or contact with travelers from those regions. 13 No US deaths from Clade I have been reported.
Clade Ib continues to circulate in multiple Western European countries through sexual networks with no international travel history in affected individuals, indicating established local transmission in that region. 13 The CDC-reported case fatality rate for Clade Ib is below 0.5%.
What you should do:

MMWR highlights

Measles hospitalization in West Texas: 1 in 5 patients required admission

MMWR Volume 75, Issue 20 (May 28) published a CDC EIS analysis of hospitalizations during the West Texas measles outbreak. 14 Among 325 confirmed cases from January 20 through March 18, 2025, 60 patients (18.5%) were hospitalized and 1 died. 14 In the 54 hospital records reviewed: 100% of patients were unvaccinated or had unknown vaccination status; 90.7% were under 18; and 88.9% had no underlying health conditions — meaning measles, not comorbidity, drove the severity. 14
The most common complication was pneumonia (72.2%), followed by dehydration (46.3%) and diarrhea (38.9%). Of those hospitalized, 70.4% needed supplemental oxygen, 7.4% went to the ICU, and 3.7% required mechanical ventilation. 14 Among the 5 hospitalized adults, 4 were pregnant women in their third trimester; 2 infants were born with measles. The full outbreak (through August 18, 2025) ultimately reached 762 confirmed cases, 99 hospitalizations, and 2 deaths. 14
The authors concluded: "Vaccination remains a critical tool for the prevention of measles infection and severe disease." 14 The 18.5% hospitalization rate aligns with the historical benchmark of roughly 1 in 5 unvaccinated measles cases requiring hospital care in the US.

Amanita mushroom poisonings in Northern California: largest outbreak in state history

The same MMWR issue reported the largest mushroom-associated hepatotoxic poisoning outbreak ever documented in California. 15 From November 18, 2025 through March 17, 2026, 39 people were poisoned in Northern California, resulting in 4 deaths (10%) and 3 liver transplants (8%). The immediate trigger was an extraordinary fruiting of Amanita phalloides (death cap, introduced from Europe) and Amanita ocreata (western destroying angel, native to California) following heavy late-November rains — a pattern the MMWR authors called a "superbloom." 15
Patients reported speaking at least six non-English languages (Spanish, Mixteco, Mam, Ukrainian, Russian, and Mandarin); 2 patients were unhoused. 15 There is no FDA-approved treatment for Amanita amatoxin poisoning; the three most commonly used agents (N-acetylcysteine, penicillin G, and silibinin) are all off-label. The prior largest California outbreak was December 2016 with 14 cases and 3 liver transplants. 15 CDPH is seeking to add Amanita poisoning to California's mandatory disease reporting list.
The authors noted: "Eating foraged mushrooms remains an activity with high risk for poisoning, especially during rainy seasons, when amatoxin-containing mushrooms can fruit widely." 15
What you should do:
  • Do not forage and eat wild mushrooms unless you have formal mycological training. Death cap and destroying angel look similar to edible species and are deadly in small quantities.
  • Clinicians: if a patient presents with severe nausea, vomiting, and delayed-onset liver failure following mushroom ingestion, contact the California Poison Control System (1-800-222-1222) or your regional poison center immediately for consultation on off-label treatment options.

Vaccines and immunization policy

No new ACIP vaccine recommendations were issued during this week. ACIP's most recent meeting was March 18–19, 2026; no meetings are scheduled before summer. 16
FDA's Vaccines and Related Biological Products Advisory Committee (VRBPAC) convened a public meeting on May 28 to advise on the 2026–2027 COVID-19 vaccine formula. 17 FDA staff presented the JN.1 lineage XFG variant as the recommended strain for the upcoming season; Pfizer/BioNTech, Moderna, and Sanofi submitted immunogenicity data in support. 17 VRBPAC's recommendations are advisory; FDA makes the final formulation decision. Updated COVID-19 vaccines for 2026–2027 are expected to be available in late summer or early fall.

Three items require closer attention:
Moringa Salmonella cluster — two separate recalls, one shared exposure source. Mogo Moringa and TNVitamins/Doctor's Pride are distinct products from different manufacturers, but both belong to a broader moringa leaf powder Salmonella contamination investigation. The TNVitamins/Doctor's Pride recall is part of a 119-case, 36-state outbreak that CDC initially closed in March 2026, then reopened on May 27 after 22 new cases across 4 additional states. 23 The Mogo Moringa recall is linked to a smaller, concurrent cluster (18 cases, 14 states). 22 Anyone who has purchased moringa leaf powder capsules online in recent months should check both brand names against their supplements regardless of the specific brand recalled.

CDI powdered milk cascade — SKS Copack is the seventh downstream brand. California Dairies Inc. (CDI) recalled approximately 2.7 million pounds of low-heat nonfat dry milk powder on April 20, 2026 after Salmonella contamination was detected. 27 SKS Copack's May 23 recall of café and restaurant specialty beverage bases across 25 states is the seventh confirmed downstream recall tied to CDI's ingredient. The CDI-linked recalls now cumulatively span caffeinated drink mixes, snack chips, nut mixes, baked goods, and specialized beverage powders. The FDA major recall center's CDI docket was last updated May 8, so additional downstream recalls remain possible. 27
Boner Bears sildenafil warning — cardiac risk. The FDA's statement in the recall notice is explicit: sildenafil "can interact with nitrates found in certain prescription drugs, such as nitroglycerin, and may lower blood pressure to dangerous levels." 20 People with diabetes, hypertension, elevated cholesterol, or heart disease face elevated risk. Anyone who consumed Boner Bears Sex Chocolate (Lot #BB21125) and takes a nitrate medication should consult a physician promptly, even if no symptoms have appeared.
E. coli beef kofta — no formal recall needed. USDA FSIS issued a public health alert rather than a recall because The Kebab Shop voluntarily removed beef kofta from all locations nationwide on May 18. 21 The 9 confirmed illnesses are all in California; 6 are children; 2 developed hemolytic uremic syndrome (HUS), a serious kidney complication associated with E. coli O157:H7. Anyone who dined at The Kebab Shop in California, Florida, or Texas and developed bloody diarrhea, severe abdominal cramps, or signs of kidney problems within 2–10 days should seek medical care. 28

  • Peanut-allergic consumers: Check packaging for SkinnyDipped Dark Chocolate Coconut Almond Bites with Best By dates December 20–29, 2026. Return to any retailer for a refund.
